The Good Samaritan Agency Bangor Maine – Post Legal Adoption Services (PLAS)
The Good Samaritan Agency assists people with the search process. If you are interested in the services, you may call the agency, 942-7211 or 1-800-249-1811 and ask to speak with Jill Holyoke, LSW.
Jill will mail you a packet of information and forms, OR download the forms, and mail the completed forms to Good Samaritan Agency, 100 Ridgewood Drive, Bangor, Maine 04401, att: Jill Holyoke.
Please note that your record can not be accessed until the following has been received:
1. Signed and notarized consent form.
2. Signed Financial Agreement
3. A letter stating what services that you want to receive.
4. Copies of two forms of identification
5. Receipt of $50 to open the record and/or additional $60 to begin the search process. If this is a hardship, please call.
Once the appropriate forms have been received, the agency can send you non-identifying information that is in the record. The agency can try to locate the person for up- dated information and/or reunion but please note that everything is confidential and non-identifying unless all parties agree.
In Maine in January 2009, the law changed and adoptees can have access to the original birth certificate by contacting the Office of Vital Statistics in Augusta. This can also be done on-line.
